Job summary

Fresenius Medical Care in Concord, NH is seeking an entry-level Registered Nurse for the Outpatient RN CAP 1 track within the Clinical Advancement Program. This role supports the ESKD care team with assessment, planning, intervention and evaluation for a defined patient group.

The position emphasizes bedside nursing, patient education, and collaboration with RNs, LPNs/LVNs and technicians, with CAP requirements to advance within the program.

Qualifications

• Graduate of an accredited School of Nursing.
• Current state nursing licensure.
• Current or successful CPR BLS Certification.
• Must meet state practice requirements.

Responsibilities

• Perform essential functions under supervision with guidance from educators and RN peers.
• Collect and analyze patient data before, during, and after hemodialysis; document in records.
• Educate patients and families on care plans and treatment options.
• Deliver safe, effective hemodialysis therapy in collaboration with care team.
• Administer medications as prescribed or by approved algorithms; document effectiveness.
• Initiate or assist with emergency response measures as needed.
• Serve as a resource for the care team and participate in staff training.
• Ensure correct lab collection, processing, and shipping; reschedule missed labs.
• Update nursing assessment and care plan in collaboration with the interdisciplinary team.
• Provide information on transplant and modality options to patients.
• Complete CAP requirements to advance within the program.
• Perform other duties as assigned.
