Falcon Pointe Post Acute of Katy, TX is seeking a licensed therapist to serve as our Outpatient Coordinator. The role involves building and strengthening relationships with outpatient referral sources, educating them on our services, and providing therapy services to long‑term care, skilled in‑patient, and outpatient settings.
Responsibilities
- Build and strengthen relationships with current and prospective referral sources by completing face‑to‑face meetings with physicians, office managers, referral coordinators, and community members.
- Identify and market to local primary and specialty physicians to address their patients’ outpatient therapy needs.
- Assist the Director of Rehab with coordinating community events, public relations campaigns, or specialty marketing campaigns.
- Manage outpatient therapy schedule to establish and maintain POC and coordinate with other therapists to reschedule any missed visits.
- Provide therapy services to long‑term care, skilled in‑patient, and outpatient patients within the facility.
- Provide therapy services in patients’ homes, group homes, ALF/ILF settings.
Requirements
- Licensed PT, PTA, OT, COTA or SLP.
- Experience in marketing in healthcare is optional, but preferred.
- Exceptional interpersonal skills, including the ability to quickly build rapport.
- Outgoing and personable.
- Willingness to become involved in local community events.
- Minimum of 1 year experience in SNF and/or home health.
- Therapist should be open to commuting to mobile outpatient therapy treatments within a 5‑mile radius of the facility.
- Therapist should be comfortable with marketing outpatient therapy to local businesses, office managers, ILF/ALF, and similar settings.

Pay rates are competitive and based on various factors.
