Job summary

AbsoluteCare is seeking a Complex Care Advance Practice Provider to join our outpatient team in Tacoma, WA. You will counsel patients on care settings, perform assessments, and provide information about specialist services within a large integrated care team.

You will manage a panel of 250–300 chronic ambulatory patients, work with social work, case management, and other professionals, and implement evidence-based PCMH principles to improve outcomes.

Qualifications

• Licensed Advanced Practice Provider in the practicing state.
• Minimum of 2 years experience as an Advanced Practice Provider.
• ACLS certification required.
• Experience with Medicare, Medicaid, and commercial populations.
• Proficiency with EMR; eClinicalWorks preferred.

Responsibilities

• Manage and improve health for patients with multiple chronic conditions (panel of 250–300).
• Collaborate with a multi-disciplinary team to implement care plans.
• Develop transitional care plans across hospital to home care.
• Coordinate with inpatient and post-acute facilities to optimize discharges.
• Educate patients on chronic diseases and healthy choices.
• Report to the Medical Director.
