AbsoluteCare Medical Center in Philadelphia seeks a Complex Care Physician Assistant, Advanced Practice Provider, to join a large, integrated care team in outpatient settings, focusing on whole-person care for patients with multiple chronic conditions.

You will manage transitions of care, coordinate with social work, case management, pharmacy, and nutrition, and support discharge planning while collaborating with supervising physicians.

Formación

• Licensed Advanced Practice Provider in the state of practice.
• 2 years of experience as an Advanced Practice Provider in Primary Care.
• DEA
• ACLS certification.
• Experience with patients who have multiple chronic medical conditions (CHF, COPD/Asthma, HTN, Diabetes).
• Experience with Medicare, Medicaid, and commercial populations.
• Knowledge of chronic disease management programs and services.
• Excellent organization and communication skills.
• Comfortable with and the ability to treat the acutely ill in an outpatient setting offering IV hydration, IV antibiotics, wound care, and other services with the collaboration of a supervising physician.
• Long-term interest in AbsoluteCare Medical Center.
• Proficiency with EMR, eClinicalWorks preferred.

Responsabilidades

• Manage and improve health for patients with multiple chronic medical conditions.
• Manage a panel of approximately 200 to 250 chronic care ambulatory patients in an office based setting.
• Work in a team environment with social work, case management, behavioral health, pharmacy, nutrition, etc., and discuss plans of care with a supervising physician.
• Develop strong transitional care plans and track paneled patients along the whole continuum of care including discharges from the hospital, ER, SNF/nursing homes, and home care.
• Communicate with alternate levels of care facilities to facilitate discharge to a lower level of care or back to AbsoluteCare Medical Center.
• Familiar with inpatient medicine and the discharge process to help facilitate discharges.
• Treat acute exacerbations within AbsoluteCare with treatments such as Lasix, IV hydration, IV antibiotics, and COPD/asthma interventions with physician oversight.
• Accept same day/next day or urgent appointments for patients from home or discharged from hospital/ER/SNF needing intensified practitioner or case management.
• Track patients and their disease processes to prevent ER visits and readmissions.
