Job summary

AbsoluteCare is seeking a Complex Care Nurse Practitioner to join our outpatient team in Philadelphia. You will assess patients, counsel on care settings, and provide information on specialist services as part of a large, integrated care team delivering whole-person care.

The role emphasizes PCMH principles, evidence-based protocols, and close collaboration with physicians, social work, case management, behavioral health, and other allied staff.

Qualifications

• Licensed Advanced Practice Provider in the state of practice.
• 2 years of experience as an Advanced Practice Provider in Primary Care.
• DEA registration and ACLS certification.
• Experience with patients with multiple chronic conditions (CHF, COPD/asthma, HTN, diabetes, renal disease).
• Experience managing Medicare, Medicaid, and commercial populations.
• Strong organization and communication skills.
• Proficiency with EMR systems, preferably eClinicalWorks.

Responsibilities

• Manage and improve health for patients with multiple chronic medical conditions.
• Handle a panel of approximately 200–250 chronic care ambulatory patients.
• Collaborate with a multidisciplinary team and supervising physician.
• Develop transitional care plans and track patients across the continuum of care.
• CoordinateDischarges and facilitate appropriate post-acute care.
• Treat acute exacerbations in the outpatient setting with physician oversight.
