Job summary

Southeast Primary Care Partners seeks an Advanced Practice Provider to join a collaborative clinic team. The APP will manage mixed outpatient care, supported by physicians, RN case managers, and staff, extending primary care services across chronic and acute conditions.

The role emphasizes a comprehensive, coordinated model of care with performance tracked against organizational metrics; outpatient focus is typical, with inpatient needs addressed as required.

Qualifications

• Graduate PA/NP program credit required.
• Current Georgia licensure required.
• Prescriptive authority through GA Board required.
• BLS certification from American Heart Association required.
• At least 2 years of primary care experience preferred.

Responsibilities

• Provide care consistent with licensure and certification.
• Diagnose and treat chronic and acute conditions in adults.
• Order and interpret lab work, X-ray, and EKG.
• Prescribe medications and treatments as needed.
• Counsel patients on disease management and treatment options.
• Collaborate with physicians and clinic staff to deliver coordinated care.
• Perform other duties as assigned.
