The role emphasizes chronic disease management and transitions of care, working with a broad care team to prevent hospitalizations, with hours about 28 per week, Monday–Friday, 8am–5pm, flexible by candidate availability.

Qualifications

• Licensed Advanced Practice Provider in the relevant state.
• Experience caring for patients with multiple chronic conditions.
• Proficiency with EMR systems (eClinicalWorks preferred).
• Strong organization and communication skills.

Responsibilities

• Manage and improve health for patients with multiple chronic conditions.
• Manage a panel of approximately 200 to 250 chronic care ambulatory patients.
• Work in a team environment with social work, case management, behavioral health, pharmacy, nutrition, etc., under a supervising physician.
• Develop transitional care plans and track patients across hospital to home care.
• Communicate with higher levels of care facilities to facilitate appropriate discharge.
• Treat acute exacerbations of chronic conditions in outpatient setting with supervision.
• Accept urgent appointments from home or hospital discharge to ensure timely care.
• Track patients to prevent ER visits and readmissions.
