Job summary

Ascension Saint Thomas Medical Partners Nashville in Nashville, TN, seeks a Pharmacist to join our primary care team on days, full-time. The role focuses on delivering tailored medication plans, real-time drug information, and safety vigilance.

You will collaborate with clinicians and mentor pharmacy students in a supportive hospital-based setting. Certification by the Tennessee Board of Pharmacy is required; a Doctor of Pharmacy degree or equivalent is expected, with outpatient dispensing

Qualifications

• Pharmacist credentialed from the Tennessee Board of Pharmacy required.
• Bachelor's or advanced degree in Pharmacy required.
• Outpatient dispensing experience preferred.

Responsibilities

• Driving clinical excellence by tailoring medication plans to the patient.
• Serving as a consultant to medical staff with real-time drug information and formulary alternatives.
• Ensuring patient safety by reviewing orders to prevent drug-related issues.
• Leading and mentoring the pharmacy team and precepting pharmacy students.

• Driving Clinical Excellence: Serving as a specialist to ensure every medication plan is perfectly tailored to the patient.
• Serving as a Consultant: Acting as a trusted advisor to medical staff and clinical leadership, providing real-time drug information and recommending formulary alternatives that prioritize safety and efficacy.
• Ensuring Patient Safety: Meticulously reviewing orders to proactively identify and resolve potential drug-related issues before they reach the bedside.
• Leading & Mentoring: Overseeing the technical pharmacy team and shaping the future of the profession by serving as a preceptor for pharmacy students.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration:

• Pharmacist credentialed from the Tennessee Board of Pharmacy obtained prior to hire date or job transfer date required.

Education

• Bachelor's or advanced degree in Pharmacy required.
