Job summary

Renown Health in Reno, NV seeks a Clinical Pharmacist-Outpatient to provide evidence-based pharmaceutical care across outpatient services. You will perform pharmacotherapy consults, medication reconciliation, and polypharmacy assessment within the retail workflow, supporting the longitudinal plan of care.

You will conduct medication monitoring, disease state management counseling, and a thorough DUR review, documenting encounters in EPIC and collaborating with physicians, patients, and care

Qualifications

• Pharmacy degree and Nevada licensure required.
• Minimum 3 years as a practicing pharmacist.
• Experience with medication therapy management and DUR review preferred.

Responsibilities

• Provide face-to-face, telephonic, or virtual pharmaceutical care to patients.
• Perform pharmacotherapy consults and disease management support.
• Conduct medication reconciliation and assess polypharmacy.
• Review drug utilization data and support formulary management.
• Document all encounters in EPIC and participate in interdisciplinary rounds.
