Job summary

Renown Health in Nevada partners with Kaiser Permanente to expand outpatient pharmaceutical care. We seek a Clinical Pharmacist-Outpatient I to provide evidence-based pharmacotherapy and engagement with patients in the outpatient setting.

You will perform pharmacotherapy consults, disease management support, medication reconciliation, and DUR reviews to optimize patient care while supporting the longitudinal plan of care. EPIC and MS Office skills are required.

Qualifications

• Bachelor of Pharmacy or Doctor of Pharmacy degree required to obtain Nevada licensure.
• 3 years as a practicing pharmacist preferred, with experience in medication therapy management.
• Active or temporary Nevada Pharmacist license required at time of hire.
• Healthcare Basic Life Support (BLS) certification within 90 days of employment.

Responsibilities

• Provide face-to-face, telephonic, or virtual pharmaceutical care to patients to promote safe, cost-effective drug therapy.
• Assist patients and physicians with medication profiles and formulary coverage to optimize therapy.
• Perform medication monitoring, disease state management counseling, and collaborative drug therapy management in EPIC.
• Develop patient education materials related to disease conditions.
• Analyze drug utilization data to identify trends and reduce drug spend across care continuum.
• Participate in interdisciplinary rounds and communicate with providers and departments.
