Mountainside Treatment Center in Canaan, CT is seeking an Outpatient Case Manager to deliver case management, crisis support, and collaborative care within our Partial Hospitalization Program. You will guide clients through goal setting, sober routine development and access to local resources.

Responsibilities include admissions, orientation, crisis interventions, and coordinating with the treatment team to maintain safety and continuity of care, while engaging families and community providers

Competenze

• Associate or Bachelor's degree in healthcare field.
• Active CASAC-T or higher required.
• Minimum two years of case management experience.
• Experience navigating substance use treatment systems and community services preferred.

Mansioni

• Provide case management sessions, check-ins and crisis support.
• Assist with goal planning and daily sober routine development.
• Facilitate admissions and orient new clients to services.
• Coordinate scheduling of PHP clients with the treatment team.
• Collaborate with family, community providers and collateral supports.
