Job summary

Mountainside Treatment Center is hiring an Outpatient Case Manager in Huntington, NY to guide clients through Partial Hospitalization Program (PHP) and coordinate therapeutic follow-up. You will conduct case management, crisis support, and family communications to support recovery goals.

Ideal candidates have CASAC-T or higher, at least two years of case management with co-occurring disorders, and familiarity with local resources to sustain wellness and transition care.

Qualifications

• CASAC-T or higher is required.
• Minimum two years of case management experience with co-occurring disorders.
• Experience navigating substance use treatment systems and community resources is preferred.

Responsibilities

• Ensure client adherence to PHP and engage in programming.
• Provide case management sessions, check-ins, and crisis support.
• Develop goals, daily routines, attendance plans, and after-care as advised.
