Job summary

Mountainside Treatment Center in New York, NY is seeking an Outpatient Case Manager to provide case management, crisis support, and therapeutic follow-up for clients in a Partial Hospitalization Program (PHP).

The role involves coordinating admissions, developing after-care plans, and collaborating with families and care teams to support sustainable recovery. Requires CASAC-T, healthcare degree, and 2+ years CM experience.

Qualifications

CASAC-T certification required.
Minimum 2 years case management experience in supporting clients with co-occurring disorders.
Degree in healthcare field required and related training.
Ability to engage clients therapeutically and coordinate care across teams.

Responsibilities

Provide case management sessions, check-ins, and crisis support.
Coordinate admissions, welcome materials, and client orientation.
Develop treatment plans, goals, and after-care planning with clients.
Maintain trauma-informed approach and professional demeanor.
Collaborate with families, community providers, and care teams to ensure continuity of care.
