Job summary

Southern Highlands Community Mental Health Center is seeking a compassionate Case Manager/Care Coordinator Level 2 for Outpatient Services in McDowell County, WV. Qualifications

Bachelor's or master's degree in a human-services-related field that meets the requirements established by the West Virginia Board of Social Work
Licensed psychologist with a master's or doctoral degree from an accredited program; or
Licensed registered nurse.

Responsibilities

Coordinate care with clients, families, treatment providers, and community organizations to support progress toward individualized treatment goals.
Advocate for client needs and promote stability, independence, and well-being.
Assist clients and families in accessing services and resources to meet care needs.
