Job summary
Southern Highlands Community Mental Health Center is seeking a compassionate Case Manager/Care Coordinator Level 2 to assist individuals in accessing behavioral health, medical, social, educational, and community resources. You will coordinate services with a multidisciplinary team to promote recovery and improve health outcomes in Mercer County.

Qualifications
- Bachelor's or Master's degree in a human services-related field per WV requirements.
- Licensed Psychologist (Master's or Doctoral) per WV standards.
- Licensed Registered Nurse.
Responsibilities
- Assess client needs and assist with service coordination.
- Develop and monitor individualized service plans.
- Connect individuals with community resources and supports.
- Advocate for clients navigating complex systems of care.
- Maintain accurate documentation per agency/state/federal rules.
- Participate in treatment team meetings with internal/external providers.
- Monitor client progress and support goal attainment.
- Promote recovery-oriented, trauma-informed, person-centered care.
