Position Summary

Southern Highlands Community Mental Health Center is seeking a compassionate, dependable, and organized Case Manager/Care Coordinator Level 2 to join our Outpatient Services team in McDowell County.

This position assists clients and families in accessing the services and resources needed to promote stability, independence, and overall well-being. The Case Manager/Care Coordinator works closely with clients, families, treatment providers, and community organizations to coordinate care, advocate for client needs, and support progress toward individualized treatment goals.

Education and Licensure Requirements

• Bachelor's or master's degree in a human-services-related field that meets the requirements established by the West Virginia Board of Social Work;
• Licensed psychologist with a master's or doctoral degree from an accredited program; or
• Licensed registered nurse.
