Job summary

Trinity Health IHA Medical Group in Michigan is seeking a compassionate Registered Nurse (RN) to join our outpatient care team. You will help patients navigate their healthcare journey through clinical triage, care coordination, patient education, and ongoing support that improves outcomes and enhances the patient experience.

Day Shift, full-time role with a focus on collaboration with providers, care managers, pharmacists, and other healthcare partners.

Qualifications

• Graduate of an accredited Registered Nurse program.
• Current, unrestricted RN license in Michigan; CPR certification.
• 1–2 years of clinical experience in a medical/physician office preferred.

Responsibilities

• Assess and triage patient needs by phone and in-office visits.
• Provide clinical guidance and patient education.
• Coordinate care and support transitions of care and post-discharge follow-up.
• Collaborate with providers, care managers, pharmacists, and behavioral health specialists.
• Manage referrals, test results, prior authorizations, and medication refills.
• Identify preventive care needs and support chronic disease management.
