Job summary

Trinity Health IHA Medical Group is seeking a compassionate Registered Nurse (RN) to join our outpatient care team. You will conduct clinical triage, provide patient education, and coordinate care to improve outcomes and patient experience.

You will assess needs by phone and in-person visits, collaborate with a broad care team, and support transitions of care and follow-up. This role emphasizes service, leadership, and community health in Southeast Michigan.

Qualifications

• Graduate of an accredited RN program.
• Active Michigan RN license and CPR certification.
• 1–2 years of clinical experience in a medical or physician office preferred.

Responsibilities

• Assess and triage patient needs by phone and in-office visits.
• Provide clinical guidance, patient education, and care coordination.
• Support patients through transitions of care and post-discharge follow-up.
• Collaborate with providers, care managers, pharmacists, behavioral health specialists, and other healthcare partners.
• Manage medication refills, test results, referrals, and prior authorizations.
• Identify preventive care needs and support chronic disease management.
• Help patients access community resources and navigate barriers to care.
• Serve as a clinical resource and leader within the care team.
