Job summary

Trinity Health IHA Medical Group is seeking a compassionate Registered Nurse (RN) to join our outpatient care team in Michigan. You will perform clinical triage, coordinate care, educate patients, and support transitions of care to improve outcomes and patient experience.

Responsibilities include triage by phone and in-office, guiding patients, managing medications and referrals, and collaborating with providers and care teams to address preventive care and chronic disease needs.

Qualifications

• Graduate of an accredited Registered Nurse program.
• Michigan RN license (valid, unrestricted) with CPR certification.
• 1–2 years of clinical experience in a medical/physician office preferred.

Responsibilities

• Assess and triage patient needs by phone and in-office visits.
• Provide clinical guidance, patient education, and care coordination.
• Support patients through transitions of care and post-discharge follow-up.
• Collaborate with providers, care managers, pharmacists, behavioral health specialists, and other healthcare partners.
• Manage medication refills, test results, referrals, and prior authorizations.
• Identify preventive care needs and support chronic disease management.
• Help patients access community resources and navigate barriers to care.
• Serve as a clinical resource and leader within the care team.
