Job summary

Intermountain Hospital in Boise is seeking a Care Navigator to meet with eligible patients and discuss outpatient services options, transportation, and scheduling to support admission and aftercare planning.

The role requires data entry into Midas, a strong grasp of inpatient workflow related to case management, and excellent computer and communication skills. Shift is 7:30 a.m.–4:00 p.m., Monday–Friday.

Qualifications

• High School diploma or GED required; Bachelor's degree preferred.
• Must have valid driver’s license, CPR and Crisis Prevention Intervention (CPR training).
• Strong computer skills with attention to detail; strong oral and written communication skills required.

Responsibilities

• Meet with eligible patients to discuss Outpatient Services benefits and logistics.
• Communicate eligibility to the Director of Outpatient.
• Routinely collect and enter data on Outpatient eligibility into Midas for trending reports.
• Understand inpatient workflow related to case management and discharge planning.
