Intermountain Health in Utah is hiring a Behavioral Health Patient Navigator to support inpatient psychiatric patients. You’ll meet referrals from inpatient units and EDs to review discharge plans and connect patients with community resources.

This role emphasizes educating patients on accessing behavioral health services, coordinating care with hospital resources, and documenting plans in patient records. Weekdays, full-time position with a shift at Intermountain Health facilities.

Formación

• Excellent interpersonal and communication skills.
• Experience adapting quickly when the need arises.
• Knowledge of available behavioral health community resources.

Responsabilidades

• Meet with patients to review discharge plans for those referred from inpatient units, EDs, and Access Centers.
• Assist patients in connecting with referred resources.
• Initiate timely follow-up calls after discharge to ensure adherence to the plan.
• Document discharge plans in the patient record.
• Gather data and complete required logs timely and accurately.
• Coordinate patient care needs with care providers and hospital resources.
• Establish and maintain an accurate list of treatment facilities and community resources.
