The Care Navigator will communicate eligibility to the Outpatient Director, collect and enter data into Midas, and support discharge planning in collaboration with inpatient staff.

Qualifications

• High School graduate or GED; Bachelor's degree preferred.
• Valid driver's license and CPR; Crisis Prevention Training optional.
• Strong computer skills and attention to detail.
• Excellent verbal and written communication.

Responsabilités

• Meet with eligible patients to discuss outpatient benefits and treatment options.
• Explain transportation, virtual programming, and start dates.
• Communicate eligibility to the Outpatient Director and document in Midas.
• Support discharge planning and care coordination with inpatient team.
