Job summary

Qualifications

• Education: High School graduate or GED; Bachelor's preferred.
• Experience: Minimum of two years in a health care environment; psychiatric/mental health experience preferred.
• Must have a valid driver's license, CPR and Crisis Prevention Intervention (training provided).
• Strong computer skills and attention to detail; strong oral and written communication skills.

Responsibilities

• Meet with eligible patients to discuss Outpatient Services, transportation options, virtual programming, and start dates.
• Communicate eligibility to the Director of Outpatient and collect data into Midas for trending reports.
• Assist with inpatient workflow understanding related to case management and discharge planning.
• Support a team of professionals dedicated to patient healing and health.
