Job summary

Qualifications

• Master's degree in social work from an accredited program.
• Bilingual skills preferred.

Responsibilities

• Provides assessment, care planning and intervention to patients and families, including psychosocial evaluation and planning.
• Offers counseling to help patients cope with illness and its social/emotional impact.
• Collaborates with providers and the health care team to coordinate care.
• Investigates insurance benefits and community resources for referrals.
• Organizes care team meetings to evaluate progress and resolve issues.
• Documents care and maintains required reporting for management systems.
