Job summary

Qualifications

• MSW or RN with PA license.
• PA license in PA or NJ required at Warren Campus.
• RN license in PA/PA reciprocity may be needed.

Responsibilities

• Develops discharge plans addressing psycho-social needs.
• Collaborates with patients, families and multidisciplinary team.
• Maintains communication with care team and coordinates resources.
• Supports patient/family adjustment and post-discharge follow-up.
