Job summary

St. Luke's University Health Network seeks an experienced Outpatient Care Manager, Registered Nurse, to provide care management services to outpatients and families in line with department policies.

The role emphasizes assessment, care planning, and coordination with the care team to support self-management and timely follow-up. The ideal candidate holds an RN license for PA/NJ, with 3+ years of direct patient care and Epic EMR proficiency.

Qualifications

• RN degree and license in PA or NJ required.
• BSN preferred.
• 3+ years of direct patient care experience required.
• Prior care management experience preferred; Epic EMR proficiency expected.

Responsibilities

• Provides assessment, care planning and intervention to outpatients and caregivers.
• Follows care management process including engagement, assessment and care plan development.
• Manages caseload and coordinates referrals with other care team members.
• Collaborates with providers, nurses, and care team to ensure comprehensive care coordination.
• Maintains accurate documentation in the electronic medical record and supports quality/improvement initiatives.
