SAC Health in San Bernardino, CA is seeking a CMT Training Specialist for the outpatient Care Management team. The role designs and executes comprehensive staff training, including onboarding, documentation, outreach, and program deliverables, with emphasis on compliance and quality.

Work week is five days, 8 hours daily, at the Brier Clinic location. The position requires MA or CHW certification, CPR/BLS, and CA driver’s license.

Qualifications

• High school diploma or equivalent required.
• Active Medical Assistant or CHW certification with CPR/BLS.
• Valid CA driver's license and auto insurance.
• Experience with SAC Care Management program preferred.
• Experience training staff in clinical settings.

Responsabilités

• Design training programs aligned with guidelines and contracts.
• Lead new employee training on chart review, enrollment, assessments.
• Coordinate external trainings and mandatory LMS modules.
• Manage EMR and Case Management access; ensure training completion.
• Provide leadership feedback on systems updates for compliance.
• Deliver skill-building sessions and monitor competency.
• Develop workflows and guidelines based on updates and contracts.
• Stay current on Care Management program details and deliverables.
• Travel to SAC Health clinics as needed; own reliable vehicle.
