Job summary

Community Health Center of Southeast Kansas, Inc. is seeking a Registered Nurse to join the clinical team.

The role centers on comprehensive nursing care within the electronic health record (EHR) system and serves as a communication link between providers and patients regarding test results, medical questions, and continuity of care. The RN will support the care team with triage, coordination of upcoming appointments, immunization status review, and liaison with hospitals and external sources to

Qualifications

Must hold a valid RN license in the practicing state.
BLS certification through American Heart Association within 6 months of hire.
Two years of outpatient clinic nursing experience preferred.

Responsibilities

Supports provider and care team by completing EHR tasks outside refills/PA; manages telephone encounters, documents, lab orders, and result notification.
Coordinates care for upcoming appointments and reviews health records for immunizations, preventive services, and tests.
Handles triage calls and assists with scheduling and care coordination between visits.
Assists diabetes program with medications, prior authorizations, DME, and related patient needs.
Performs other duties as assigned; participates in daily huddles and maintains confidentiality per HIPAA.
