Chatham Hills Subacute Care Center is seeking an Occupational Therapist to join our growing team in Mountainside, NJ. The OT will assess referred patients, develop and manage treatment programs, and document patient progress while educating families about therapy options.

Must have graduated from an accredited OT program with a valid license. Experience in geriatric or long-term care and supervision of assistants is preferred. Strong professionalism and patient-centered care are essential.

Formación

• Graduated from an accredited Occupational Therapy program.
• Valid state OT license and registration.
• Experience supervising Occupational Therapy Assistants.
• Experience in long-term care or geriatric care preferred.
• Strong documentation writing skills.
• Experience with evaluations, progress notes, re-certification and discharge documentation.
• Collaborates with all disciplines to provide patient-centered care.
• Exceptional patient service skills and clinical excellence.
• Punctual and professional.
• Complies with HIPAA and confidentiality.

Responsabilidades

• Assess referred patients for rehabilitation and determine need for therapy.
• Provide direct patient care and establish a treatment plan with goals.
• Administer and manage rehabilitation treatment under Physician direction.
• Document patient progress and educate patients and families on therapy methods.
