Evolve Therapy Services, LLC is seeking a Certified Occupational Therapy Assistant to support an in-house Skilled Nursing Facility in Dyersburg, Tennessee. This PRN position focuses on delivering high-quality occupational therapy services under supervision, aligned with CMS standards.

You will schedule and perform therapy sessions, document progress, and collaborate with a multidisciplinary team to ensure resident-centered care and compliance with regulatory guidelines.

Qualifications

• Associate's degree in Occupational Therapy required.
• Active OTA licensure in state(s) of practice required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention based on scope of practice.
• Perform treatment plans and follow goals per plan of care.
• Interpret and communicate evaluations, plans, and recommendations to residents and families.
