Evolve Therapy Services, LLC in Church Hill, TN seeks an Occupational Therapist for an in-house Skilled Nursing Facility in a PRN capacity. You will deliver OT services per current standards to ensure high-quality resident care.

Responsibilities include screenings, developing treatment plans with goals, communicating findings to families and the team, scheduling therapy, supervising COTA, and ensuring proper documentation and billing. Licensure and ongoing education are required.

Qualifications

• Requires degree in Occupational Therapy.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education to maintain licensure.

Responsabilités

• Conduct regular screenings to determine best intervention for residents.
• Evaluate residents to develop treatment plans with short- and long-term goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned and as required to effectively treat patients.
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed.

Formation

Degree in Occupational Therapy

Active Occupational Therapy license

Continuing education to maintain licensure
