Job summary

Evolve Therapy Services, LLC is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Newport, TN. This PRN role supports the therapy team, evaluating residents and delivering planned OT services.

Responsibilities include screening, treatment implementation, documentation, and collaboration with an interdisciplinary team to ensure quality resident care. Active state licensure is required and ongoing CE is encouraged.

Qualifications

• Associate degree in Occupational Therapy is required.
• Active OTA licensure in state(s) of practice is mandatory.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention based on scope of practice.
• Implement treatment plans and monitor progress toward goals with supervising therapist.
• Communicate evaluation findings and plans to residents, families and care team.
• Schedule resident therapy in coordination with team members.
• Document and bill occupational therapy services per plan of care.
• Perform additional duties to support patient care.
• Ensure CMS documentation standards are followed daily.
