Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant to provide in-house skilled nursing facility services in Humboldt, TN. This PRN role supports plan of care implementation, resident evaluation, and collaboration with the multidisciplinary team to maintain high-quality care.

You will conduct screenings, implement treatment plans, document outcomes per CMS standards, and coordinate schedules with staff while pursuing ongoing professional development and licensure.

Qualifications

• Must have OTA licensure in applicable state(s).
• Associate degree in Occupational Therapy.
• Ongoing continuing education to maintain licensure and certification.

Responsabilités

• Screen residents to determine best intervention.
• Perform treatment plans and follow short- and long-term goals.
• Interpret and communicate evaluation findings to residents and families.
• Establish resident therapy schedules with the team.
• Treat according to plan and document outcomes and billing.
• Perform all other duties to effectively treat patients.
• Comply with CMS documentation standards daily.
