Job summary

Creative Solutions in Healthcare - East seeks a compassionate Occupational Therapy Assistant (OTA) to join the in-house rehab team. You will provide person-centered care to residents to improve independence and quality of life under the supervision of an OT.

The ideal candidate holds a Texas OTA license, a COTA credential, and SNF/long-term care experience. Knowledge of Net Health EDS is a plus; strong communication and teamwork are essential for coordinating care.

Qualifications

• Texas Occupational Therapy Assistant license in good standing.
• Certified Occupational Therapy Assistant (COTA) credential required.
• Strong communication and teamwork abilities.
• Experience in SNF or long-term care settings preferred.
• Knowledge of occupational therapy treatment practices and documentation.

Responsibilities

• Plan and administer OTA treatments to residents to relieve pain and improve function.
• Provide skilled services under the supervision of an OT per physician orders.
• Relate service requests to the OT and relay information promptly.
• Deliver treatments per the care plan and ensure timely documentation.
• Educate patients and caregivers and monitor responses to therapy.
• Attend required Rehab Director meetings and maintain regulatory-compliant records.
