Job summary

Enhance Rehabilitation is seeking qualified Occupational Therapist candidates to join our growing team. You will evaluate and treat patients in a SNF setting following an individualized plan of care.

You will provide excellent patient care, collaborate with facility staff to ensure optimal outcomes, develop and adjust plans of care, educate patients on their injury, progress and probable outcomes, complete timely documentation, and supervise Occupational Therapy Assistants.

Responsibilities

• Evaluating new patients, developing plans of care and setting measurable, functional goals.
• Treating patients, addressing concerns and educating them on injury, progress and outcomes.
• Re-evaluating patient progress, revising plans of care and resetting goals.
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
• Providing excellent care for our patients.
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for your patients
