Job summary

Enhance Rehabilitation is seeking qualified Occupational Therapist candidates to join our dynamic team in a SNF setting. You will evaluate and treat patients in this setting following an individualized plan of care.

You will provide excellent care and service to patients and families, collaborate with facility staff, and document progress and outcomes while supervising Occupational Therapy Assistants.

Qualifications

• Licensed Occupational Therapist with ability to evaluate patients.
• Develop plans of care and set measurable, functional goals.
• Provide high-quality patient care and customer service.
• Supervise Occupational Therapy Assistants.
• Collaborate with facility staff to ensure excellent care.

Responsibilities

• Evaluate new patients and develop plans of care.
• Treat patients and educate them on injury, progress and outcomes.
• Re-evaluate patient progress and adjust plans as needed.
• Complete timely and accurate documentation (evaluations, re-evaluations, discharges, progress notes).
• Supervise Occupational Therapy Assistants and coordinate with the team.
