Concept Rehab is seeking a licensed Occupational Therapist to join our skilled nursing and long-term care rehabilitation team in Cincinnati. You will assess patients, develop individualized treatment plans, and deliver skilled OT interventions to improve daily living activities and mobility.

Qualifications

• Bachelor's, Master's, or Doctorate Degree in Occupational Therapy.
• Current OT license in the state of practice.
• Strong communication and interpersonal skills.
• Ability to work independently and within a collaborative healthcare team.
• Commitment to patient-centered care and clinical excellence.

Responsibilities

• Assess patients' functional abilities and establish individualized OT treatment plans.
• Provide skilled interventions to improve daily living activities, strength, coordination, cognition, mobility, and functional performance.
• Evaluate patient response to treatment and modify interventions as needed.
• Supervise OTAs in accordance with regulations and standards.
• Collaborate with physicians, nursing, families, and the care team.
• Contribute to care conferences, care planning, and discharge planning.
• Educate patients and caregivers on strategies to promote safety and independence.
• Identify adaptive equipment and interventions to support outcomes.
• Complete clinical documentation and billing within set timeframes.
• Maintain compliance with licensure requirements and OT practice standards.
