Concept Rehab is seeking a dedicated Occupational Therapist (OT) to join our therapy team at a Skilled Nursing facility in Kettering and Centerville, Ohio. This full-time role offers flexible scheduling, comprehensive benefits, and professional growth within a therapist-owned organization.

Qualifications

• Bachelor's, Master's, or Doctorate Degree in Occupational Therapy.
• Current Occupational Therapist license in the state of practice.
• Strong communication and interpersonal skills.
• Ability to work independently and within a collaborative healthcare team.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide skilled occupational therapy services.
• Help patients improve daily living skills, mobility, cognition, strength, and functional independence.
• Monitor progress and adjust treatment plans as needed.
• Supervise Occupational Therapy Assistants (OTAs).
• Collaborate with physicians, nurses, families, and interdisciplinary team members.
• Participate in care plan and discharge planning meetings.
• Educate patients, caregivers, and facility staff.
• Recommend equipment and interventions to improve patient outcomes.
• Maintain timely, accurate documentation and billing.
• Comply with all state licensure, regulatory, and professional practice standards.
