Job summary

Concept Rehab, Inc. is seeking a licensed Occupational Therapist to join our PRN rehabilitation team in a Skilled Nursing Facility (SNF) setting. You will assess and treat patients, develop individualized plans, and collaborate with an interdisciplinary team to support ADLs and mobility.

Ideal candidates will have strong clinical judgment, excellent communication, and be able to work independently while contributing to discharge planning and family education in a fast-paced rehab environment.

Qualifications

• Bachelor's, Master's, or Doctorate in Occupational Therapy.
• Current Occupational Therapist license in the state of practice.
• Ability to work independently while collaborating with an interdisciplinary team.
• Strong communication, documentation, and clinical decision-making skills.

Responsibilities

• Perform occupational therapy evaluations within 24-48 hours of physician orders.
• Develop and implement individualized treatment plans.
• Provide skilled occupational therapy services in accordance with physician orders and established plans of care.
• Evaluate and treat deficits related to ADLs, functional mobility, cognition, strength and endurance, positioning and safety.
• Supervise OTAs in accordance with state practice acts and company policy.
• Collaborate with physicians, nursing staff, patients, families, and interdisciplinary team members.
• Participate in care plan meetings and discharge planning.
• Complete accurate documentation, progress notes, evaluations, discharge summaries, and daily billing.
• Educate patients, caregivers, and facility staff on therapy goals and maintenance programs.
• Recommend adaptive equipment and environmental modifications to enhance patient outcomes.
