Job summary

Innovate Rehab and Wellness seeks a Licensed Certified Occupational Therapy Assistant (PRN) to join our Skilled Nursing Facility team. Deliver high-quality OT services, help residents regain independence, and document progress under OT supervision.

Requirements include an Associate’s degree in OTA, NBCOT certification, current state licensure, and strong communication. SNF experience is preferred; you will collaborate with nursing, PT, SLP, and other staff to ensure patient-centered rehab

Qualifications

• Associate’s degree from OTA program required.
• NBCOT certification required.
• Current state license as an Occupational Therapy Assistant required.

Responsibilities

• Implement treatment plans created by the Occupational Therapist.
• Provide direct patient care per state and facility guidelines.
• Monitor and document resident progress; report changes to the OT.
• Educate residents and caregivers on adaptive techniques and equipment.
• Collaborate with nursing, PT, SLP, and other staff.
• Maintain accurate documentation in the EMR.
• Participate in care planning and discharge planning.
• Ensure compliance with HIPAA and CMS regulations.
