Evolve Therapy Services, LLC is hiring a Certified Occupational Therapy Assistant to provide in-house OT services at a Skilled Nursing Facility in Olivia, MN. You will help plan, implement, and document patient care under supervision to maintain high quality resident outcomes.

The role requires an OTA license and an Associate's degree in Occupational Therapy, plus ongoing professional development. Full-time, part-time, or PRN options with a $5,000 sign-on and tuition reimbursement.

Qualifications

• Associate's degree in Occupational Therapy.
• Active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Plan, organize, develop, and direct Occupational Therapy Services per facility standards.
• Conduct resident screenings to determine best intervention or treatment.
• Implement treatment plans and follow short- and long-term goals with supervision.
• Communicate evaluation findings and recommendations to residents, families, and interdisciplinary team.
• Establish therapy schedules and ensure documentation and billing of OT services.
• Comply with CMS documentation standards and maintain professional conduct.
