Job summary

Evolve Therapy Services, LLC is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility partner located in Olivia, MN. The role focuses on delivering planned occupational therapy services under supervision to residents.

The incumbent will screen residents, implement treatment plans, document outcomes, and collaborate with a multidisciplinary team. Full-time positions with competitive pay and room for growth.

Qualifications

• Associate's degree in Occupational Therapy.
• MUST have active OTA licensure in state(s) of practice
• Ongoing continuing education and professional development to maintain current licensure and certification

Responsibilities

• Screen residents to determine best intervention
• Perform treatment plans under supervising therapist and follow goals
• Interpret and communicate evaluation findings to residents and families
• Establish individual resident therapy schedules with team
• Document and bill occupational therapy services
• Perform other duties as assigned to treat patients
• Comply with CMS documentation standards of practice daily
