Job summary

Evolve Therapy Services is hiring a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Olivia, MN. This full-time role offers a $5,000 sign-on plus tuition reimbursement and flexible PT/PRN options.

The OTA will plan, organize, develop, and direct Occupational Therapy Services in accordance with federal, state, local standards, and administrator directives to ensure high-quality resident care.

Qualifications

• Associate's degree in Occupational Therapy (required).
• OTA licensure active in practice states.
• Commitment to ongoing continuing education.

Responsibilities

• Conduct regular screening of residents to determine interventions.
• Implement treatment plans and follow short- and long-term goals.
• Communicate evaluation findings and recommendations to residents and care teams.
• Establish therapy schedules with team members.
• Document and bill occupational therapy services.
• Perform other duties to treat patients as needed.
• Comply with CMS documentation standards daily.
