Job summary

Evolve Therapy Services, LLC is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility located in Olivia, MN. This full-time role includes patient evaluation, treatment planning, and interdisciplinary coordination to ensure high-quality resident care.

You will establish therapy schedules, document sessions for CMS compliance, and collaborate with supervising therapists, families, and the care team to meet short- and long-term goals.

Qualifications

• Must have active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities
