Job summary

Evolve Therapy Services in Olivia, MN is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility. This full-time role offers a $5,000 sign-on bonus and tuition reimbursement, with additional Part Time or PRN positions available.

You will plan, organize, and direct occupational therapy services, screen residents, implement treatment plans, document and bill according to CMS standards, and collaborate with the interdisciplinary team to maintain high-quality

Qualifications

• Associate's degree in Occupational Therapy.
• MUST have active OTA licensure in state(s) of practice
• Ongoing continuing education and professional development to maintain licensure and certification

Responsibilities

• Screen residents to determine best intervention for each patient.
• Perform treatment plans and follow established short- and long-term goals.
• Interpret and communicate evaluation findings to residents, families/caregivers, and the interdisciplinary team.
• Establish individual resident therapy schedules with other team members.
• Conduct therapy according to the treatment plan and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards of practice daily with point-of-care documentation as indicated

Education

Associate's degree in Occupational Therapy

OTA licensure (active) in state(s) of practice
