Job summary

Creative Solutions in Healthcare, Inc. seeks a compassionate Occupational Therapy Assistant (OTA) to join our in-house rehab team. You will deliver person-centered care to help residents improve independence and quality of life, under the supervision of an Occupational Therapist.

Responsibilities include planning and administering treatments, documenting thoroughly, and collaborating with patients, families, and caregivers. Texas licensure and COTA credential are required.

Qualifications

• Active Texas Occupational Therapy Assistant license.
• Certified Occupational Therapy Assistant (COTA) credential required.
• In good standing with regulatory agencies and licensing boards.
• SNF or long-term care experience preferred.
• Knowledge of occupational therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.
• Ability to manage patients with varying personalities and needs.
• Strong communication and teamwork skills.

Responsibilities

• Plan and administer occupational therapy treatments to residents and patients with disabilities.
• Provide skilled occupational therapy services under supervision of an OT.
• Respond to requests for service and relay information to the OT
• Ensure treatments align with the care plan.
• Provide clinical support and instruction to patients, families, and caregivers.
• Monitor patient response and report changes.
• Complete required documentation per policy and regulations.
• Attend required meetings as designated by the Director of Rehab.
