Job summary

Creative Solutions In HealthCare is seeking a Full-Time Occupational Therapy Assistant (OTA) to join our in-house rehab team. You will provide person-centered care to residents in our skilled nursing facility and help improve independence and quality of life.

Under OT supervision, you will plan and administer treatments, document per policy, and collaborate with caregivers. A Texas OTA license and COTA credential are required; new graduates are welcome to apply.

Qualifications

• Active and valid Texas Occupational Therapy Assistant license.
• COTA credential required.
• SNF or long-term care experience preferred.
• Current knowledge of occupational therapy treatment practices.
• Experience with Net Health Electronic Documentation System.

Responsibilities

• Plan and administer occupational therapy treatments to residents.
• Provide services under supervision of an Occupational Therapist.
• Respond to requests for service and coordinate referrals.
• Ensure treatments follow the established plan of care.
• Provide instruction to patients, families, and caregivers.
• Monitor patient response and report changes.
• Complete required documentation thoroughly and timely.
• Attend required meetings designated by Rehab Director.
