Job summary

Evolve Therapy Services, LLC seeks a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Oklahoma City, to plan, direct and supervise OT services in accordance with federal and state standards, and as directed by the Administrator to ensure the highest quality resident care.

To contribute to a collaborative interdisciplinary team, establish resident therapy schedules, and implement treatment plans while maintaining accurate documentation and billing for OT

Qualifications

• Associate's degree in Occupational Therapy.
• Active OTA licensure in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Conduct regular screening of residents to determine best intervention.
• Perform treatments per established plan and goals.
• Interpret and communicate evaluation findings to residents, families and team.
• Establish resident therapy schedules with team members.
• Deliver therapy and ensure proper documentation and billing.
• Comply with CMS documentation standards daily.
• Perform other duties as assigned to support resident care.
