Job summary

Evolve Therapy Services, LLC is seeking an Occupational Therapist for an in-house Skilled Nursing Facility in Oklahoma City, OK. This role supports full-time, part-time, or PRN schedules, collaborating with an interdisciplinary team to deliver high-quality resident care.

The ideal candidate holds an OT degree and an active license, with commitment to ongoing professional development and accurate documentation for reimbursement.

Qualifications

• Must hold an Active Occupational Therapy license in practicing state(s).
• Degree in Occupational Therapy from an accredited program.
• Commitment to ongoing continuing education.

Responsibilities

• Screen residents to determine best OT interventions.
• Evaluate residents and develop treatment plans with goals.
• Communicate evaluation findings to residents, families, and team.
• Establish individual therapy schedules with the team.
• Deliver therapy per plan and document and bill services.
• Oversee COTA and ensure compliant documentation and reimbursement.
• Carry out additional duties as needed.
