Job summary

Evolve Therapy Services is seeking a licensed Occupational Therapist to oversee in-house therapy services at a Skilled Nursing Facility in Oklahoma City, OK. The role offers Full Time, Part Time, or PRN arrangements and involves planning, executing, and supervising OT interventions with an emphasis on quality resident care.

You will evaluate residents, develop treatment plans with measurable goals, collaborate with the interdisciplinary team, document care, and ensure compliant billing

Qualifications

• Must hold an active Occupational Therapy license in the practicing state.
• Degree in Occupational Therapy.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Screen residents to determine best OT intervention.
• Evaluate residents and develop treatment plans with goals.
• Communicate findings to residents, families, and the care team.
• Create therapy schedules with the team.
• Provide therapy per plan and handle documentation and billing.
• Oversee COTA as needed and ensure compliant care.
